Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform comprehensive in-home health assessments. Visits typically last up to one hour and may be conducted in members’ homes or Skilled Nursing Facilities.

You’ll review medical history, medications, and health risks to close care gaps and improve outcomes. Offerings include flexible scheduling, competitive pay based on assessments completed, and opportunities for licensure support across states.

Qualifications

• Master’s Degree or commensurate experience and NP licensure completed.
• Current NP licensure in the state of practice.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage for the state of residence.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric NP assessments including medical history, diagnosis and treatment, health education, referrals, follow-up and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other care team members.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (home or SNF).
