Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform Comprehensive Health Assessments in members' homes or SNFs. You’ll review medical history, medications, and risk factors, then coordinate care with physicians and care teams to close gaps in health outcomes.

Providers work with a supportive team, enjoy flexible scheduling, and can advance with licensure support. This role emphasizes reliability, strong communication, and patient education to improve community health.

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice to include prescription authority or ability to obtain prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience preferred, new grads encouraged to apply.
• Valid state driver's license for the state of residence.
• Automobile insurance coverage for the state of residence.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult / geriatric assessments including medical history, diagnosis and treatment, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other care team members.
• Coordinate with PCP on patient education and follow-up as necessary.
• Provide services at the member’s residence, including homes or Skilled Nursing Facilities.
