Job summary

Matrix Medical Network seeks experienced nurse practitioners to perform Comprehensive Health Assessments in patients' homes, including medical history, diagnoses, and education. You will order tests and coordinate with clinicians to close care gaps.

This role emphasizes collaboration with PCPs, case managers, and families while delivering high-quality care in patients' residences, including SNFs, with flexible scheduling and competitive compensation.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice
• Board certification in one of Adult/Primary Care specialties
• Current BLS, ACLS or CPR cert
• 1 year NP experience preferred
• Valid state driver's license
• Auto insurance coverage or ability to obtain

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnosis
• Administer and order point-of-care and lab tests
• Collaborate with physicians, case workers, and family members
• Provide in-home services at member’s residence (home or SNF)
• Maintain documentation per Matrix guidelines
