Matrix Medical Network seeks a Nurse Practitioner to conduct comprehensive in-home assessments, reviewing health history, medications and social factors to identify care gaps. You will administer tests, coordinate with PCPs and care teams, and provide care at patients’ homes or facilities.

The role emphasizes compassionate, person-centered care with a flexible schedule and opportunities for ongoing education and licensure support.

Formación

• Master’s Degree in Nursing or commensurate experience with NP licensure
• Current NP licensure in state of practice with prescriptive authority or ability to obtain
• Board certification in Adult, Adult Gerontology, Family, Emergency or Acute Care specialties
• Current BLS/ACLS/CPR certification
• 1 year NP experience preferred, new grads encouraged to apply
• Valid state driver’s license
• Automobile insurance coverage required or ability to obtain
• Strong computer skills and familiarity with health/medical record software
• Excellent verbal and written communication skills

Responsabilidades

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education and referrals
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers and caregivers
• Coordinate with PCP on patient education and follow-up
• Provide services at members’ residences (homes or Skilled Nursing Facilities)
