Job summary

Matrix Medical Network is hiring Nurse Practitioners to conduct comprehensive health assessments at members’ homes, reviewing medical history, medications, and risks during visits up to one hour. You will work with physicians, case managers, social workers, and family members to close care gaps and improve outcomes.

Qualifications

• Must hold NP license in the state of practice.
• Current national or state board certification in a relevant NP specialty.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid driver’s license and auto insurance in home state.
• Excellent communication and documentation skills.

Responsibilities

• Perform adult/geriatric health assessments in member homes.
• Review medical history, medications, and risk factors with documentation.
• Coordinate with PCPs, case managers, and caregivers for care gaps.
• Order/perform point-of-care tests and lab work as needed.
• Provide education and follow-up to support health outcomes.
