Job summary
Matrix Medical Network is hiring nurse practitioners to perform Comprehensive Health Assessments in patients’ homes across the United States. Providers review health histories, medications, and risks, and coordinate follow‑ups with physicians and care teams.

Qualifications
- Master’s Degree in nursing or commensurate NP experience.
- Current NP licensure and prescriptive authority as needed.
- Board certification in Adult/Family/ gerontology acute care as applicable.
- BLS/ACLS/CPR certification.
- Valid driver’s license and auto insurance where required.
- Strong computer and EMR literacy.
- Excellent verbal and written communication skills.
Responsibilities
- Conduct in‑home comprehensive health assessments reviewing medical history, meds, and risks.
- Order and administer point‑of‑care and lab tests as needed.
- Collaborate with PCPs, case managers, social workers, and caregivers.
- Provide services at members’ residences, including homes and SNFs.
- Document findings clearly per Matrix guidelines and facilitate follow‑ups.
