Job summary

Matrix Medical Network seeks a Nurse Practitioner to conduct comprehensive in-home health assessments, reviewing medical history, medications, and social factors to improve care gaps. You will collaborate with physicians and care teams, and deliver services at members’ homes or SNFs.

The role emphasizes flexible scheduling, clinical autonomy, and ongoing education support while enabling meaningful patient impact across communities.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in relevant NP specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply!
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage requirements for the state of residence.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education, referrals, follow-up, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other care team members.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
