Job summary

Matrix Medical Network seeks a qualified Nurse Practitioner to perform at-home comprehensive health assessments, reviewing health history, medications, and risks during visits at members' residences.

Providers conduct assessments, order tests as needed, collaborate with physicians and care teams, and help close care gaps with flexible scheduling. Full-time opportunities include onboarding support and continuing education.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice
• AANP, AACN, or ANCC board certification in Adult/AGPC/Family specialties
• Current BLS, ACLS or CPR certification
• 1 year NP experience preferred, new grads encouraged to apply
• Valid state driver's license
• Auto insurance requirements for residence (or ability to obtain)

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education, referrals, and documentation
• Administer and order point of care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, and caregivers
• Coordinate with PCP on education and follow-up
• Provide services at member residences (homes or SNFs)

Skills

NP licensure in state practice
