Job summary

Matrix Medical Network seeks experienced NP providers to perform Comprehensive Health Assessments in members’ homes. You will review health history, medications and risks, conduct assessments and coordinate with physicians, case managers and caregivers.

Positions offer flexible scheduling, travel to member homes or SNFs, and competitive compensation with licensure support. Prior NP licensure and board certification are required.

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in your practicing state with prescriptive authority if needed.
• Board certification in Adult, Gerontology, Family or Emergency NP (ANCC/AANP/AACN).
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance as required by state of residence.
• Strong computer skills and familiarity with medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnoses and referrals.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Educate patients and coordinate follow-up with PCPs.
• Provide services at members’ homes or Skilled Nursing Facilities.
