Matrix Medical Network seeks Nurse Practitioners to perform comprehensive in-home health assessments. You will review medical history, medications, and social factors, and coordinate with PCPs to close care gaps.

Providers work in members' homes or SNFs, with flexible scheduling and opportunities for full-time roles and sign-on bonuses. Competitive compensation and ongoing education support are offered.

Formación

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• Board certification in Adult, Adult Gerontology, Family, or Emergency nursing.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsabilidades

• Conduct adult/geriatric assessments including medical history and documentation per Matrix guidelines.
• Administer and order point of care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and other healthcare professionals.
• Educate patients and follow up with PCPs on care plans and referrals.
• Provide services at the member’s residence, including homes or Skilled Nursing Facilities.
