Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history, medication adherence, and risk factors. Assessments occur at patients' homes or Skilled Nursing Facilities, with collaboration across care teams.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult, AGPC, Family or related specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnoses.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP for patient education and follow-up.
• Provide in-home or SNF-based health assessments.
