Job summary

Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes, typically lasting up to an hour. You review health history, medications, adherence, and social factors to identify risks and care gaps.

Providers collaborate with primary care physicians and care teams, may order tests, and travel to members' residences including SNFs. Flexible scheduling, Sundays–Saturdays, 8 am–7 pm; full-time roles with potential sign-on bonuses.

Qualifications

• Must have NP licensure in state of practice.
• Current BLS, ACLS or CPR certification.
• Valid state driver’s license and auto insurance.
• 1 year NP experience preferred; new grads welcome.
• Ability to obtain prescriptive authority.
• Strong clinical assessment and documentation skills.
• Comfort working in patients' homes or SNFs.

Responsibilities

• Conduct adult/geriatric health assessments at home and document findings.
• Review medical history, medications, and adherence; identify gaps.
• Coordinate with PCP and care team for referrals and follow-up.
• Order tests (labs/point-of-care) as needed and interpret results.
• Provide patient education and care planning with families.
