Job summary

Matrix Medical Network seeks qualified nurse practitioners to perform comprehensive in-home health assessments. Each visit lasts about one hour and focuses on medical history, medications, and social factors that affect health outcomes.

Providers administer tests as needed and coordinate with PCPs, nurses, and care teams to close care gaps. Benefits include flexible scheduling and opportunities for continuing education and licensure support.

Qualifications

• Master’s degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice and prescriptive authority as needed.
• Board certification in adult/gerontology/family or acute care specialties.
• CPR/BLS/ACLS certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatments.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
