Job summary

Matrix Medical Network in the United States seeks a Nurse Practitioner to perform in-home comprehensive health assessments. You will review health history, current medications, and social factors, delivering care where members live.

This role offers flexible scheduling, pay per assessment, and opportunities for continued education, with travel across states and potential transitions to full-time roles, including support for licensure and credentials.

Qualifications

• Master’s degree or equivalent nursing experience with NP licensure.
• Current NP license in practicing state with prescriptive authority.
• Board certification in an NP specialty (Adult, Gerontology, Family, etc.).
• Current BLS/ACLS or CPR certification.
• Minimum 1 year NP experience.
• Willingness to travel up to 80–90% including overnight stays.
• Valid driver’s license and auto insurance coverage.
• Strong computer skills and EMR experience.
• Excellent verbal and written communication.

Responsibilities

• Conduct adult/geriatric assessments with medical history, diagnosis and treatment.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and family members.
• Educate patients and follow up with PCP as necessary.
• Provide services at member homes or Skilled Nursing Facilities.
