Matrix Medical Network seeks qualified Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes or SNFs. You'll review medical history, medications, and social factors, document findings, and coordinate with physicians and care teams.

Requirements include NP licensure in your state, board certification in Adult/Gerontology/Family, BLS/ACLS, 1 year of NP experience preferred, and a valid driver's license with auto insurance.

Formación

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice to include prescription authority or ability to obtain.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver's license with automobile insurance coverage.

Responsabilidades

• Conduct adult/geriatric health assessments including history, diagnosis and treatment planning, education and referrals.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers and care teams and document according to Matrix guidelines.
