Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, documenting medical history, medications, and risk factors. You will coordinate with physicians and care teams to close gaps in care, provide education, and arrange referrals.

Roles include home visits at member residences or SNFs, with flexible scheduling. The position requires active NP licensure, state-prescribed prescriptive authority, and board certification in relevant specialties.

Formación

• Must hold NP licensure in the state of practice and prescriptive authority as needed.
• AANP/AACN/ANCC board certification in Adult, Gerontology, Family, Emergency or Acute Care.
• BLS/ACLS or CPR certification required.
• Driver’s license and automobile insurance coverage for residence state.

Responsabilidades

• Perform adult/geriatric health assessments including medical history, diagnoses, treatment, education, referrals, and documentation.
• Order and administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers to coordinate care.
• Coordinate patient education and follow-up with PCPs.
• Provide services at members' homes or Skilled Nursing Facilities as appropriate.
