Matrix Medical Network seeks a Nurse Practitioner to conduct in-home health assessments for members across the United States. You’ll review health history, medications, and social factors to inform care plans, with coordination to physicians and case managers.

In-home visits may occur in residences or Skilled Nursing Facilities. Experience as an NP with active licensure and prescriptive authority is required; board certification in relevant specialties is preferred.

Formación

• Master’s Degree or commensurate experience with NP licensure in practice state.
• Current NP licensure and prescriptive authority or ability to obtain.
• Board certification in Adult/Adult Gerontology/Family or related specialty.
• CPR/BLS/ACLS certification current.
• 1 year NP experience preferred; new grads welcome.

Responsabilidades

• Perform adult/geriatric assessments including history, diagnosis, education and referrals.
• Order and administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and family caregivers.
• Coordinate with PCP for education and follow-up.
• Provide in-home services at members' residences or SNFs.
