Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home assessments, reviewing health history, medications, and risk factors at the member’s residence. The role emphasizes patient-centered care with documentation and coordination with PCPs and care teams.

Providers will conduct assessments at patients’ homes or SNFs, with flexible scheduling across weekends and various hours. Prior NP licensure and board certification are required, with potential full-time

Qualifications

• Master’s NP degree and licensure in practicing state.
• Current NP licensure with prescriptive authority as needed.
• Board certification in relevant NP specialty (Adult, Family, etc.).
• BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcomed.

Responsibilities

• Perform adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and care teams.
• Coordinate with PCP on education and follow-up as necessary.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
