Job summary

Matrix Medical Network seeks a highly experienced Nurse Practitioner to perform Comprehensive Health Assessments in patients’ homes or SNFs across the United States. You’ll review medical history, medications, and social factors, then coordinate with PCPs and care teams to close care gaps.

Responsibilities include conducting geriatric assessments, ordering tests, documenting findings, and supporting ongoing patient education. A valid NP license and state prescriptive authority are required.

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice to include prescription authority or ability to obtain prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.

Responsibilities

• Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols.
• Administer and order point of care and lab-based testing as needed.
• Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary.
• Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities.
