Job summary

Matrix Medical Network seeks nurse practitioners to perform in-home comprehensive health assessments for adults and seniors, reviewing medical history, current health, and medication adherence to close care gaps. Visits occur at the member's home or SNFs and are scheduled to fit your availability.

Providers administer tests, collaborate with PCPs, and deliver education and follow-up. A state NP license, board certification, and access to prescriptive authority are required.

Qualifications

• Master’s degree or equivalent and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult/Adult Gerontology/Family/ACNP/ACCS/ED/ Acute Care.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver's license.
• Auto insurance coverage as required by state (or ability to obtain).
• Strong computer skills and familiarity with health record software.
• Excellent verbal and written communication.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, referrals, follow-up, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other care team members.
• Coordinate with PCP on patient education and follow-up.
• Provide services at member homes or Skilled Nursing Facilities.
