Job summary

Matrix Medical Network seeks skilled nurse practitioners to perform Comprehensive Health Assessments in members' homes or SNFs. Each visit reviews health status, history, adherence, and social factors to close care gaps and improve outcomes.

The role requires NP licensure in the practice state with prescriptive authority where needed. Licensure board certification, BLS/ACLS, and a valid driver’s license are essential, with travel and flexible scheduling expected.

Qualifications

• Master’s degree in nursing or commensurate experience and NP licensure.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, follow-up and documentation according to Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers; follow up with PCP education.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
