Job summary

Matrix Medical Network seeks NP providers to perform Comprehensive Health Assessments in members' homes or SNFs nationwide. Each visit can last up to one hour and covers medical history, medication adherence, social factors, and health risks to close care gaps.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or eligible.
• Board certification in an approved Adult/Family/GERIATRIC NP specialty.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, treatments, and health education.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers; provide documentation per Matrix guidelines.
• Coordinate with PCP for patient education and follow-up.
