Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes, including medical history review, risk assessment, and care planning. Assessments can last up to one hour and may occur at homes or Skilled Nursing Facilities.

Providers will administer point-of-care testing, work with multi-disciplinary teams, and document findings per Matrix guidelines. In-person visits are scheduled based on availability, with opportunities for FT roles and

Qualifications

• Master’s degree in nursing practice and NP licensure.
• Current NP licensure in state of practice with prescriptive authority (if needed).
• Board certification in Adult/Adult Gerontology or Family NP specialties (AANP/AACN/ANCC).
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome to apply.

Responsibilities

• Perform adult/geriatric health assessments including medical history, diagnoses, and follow-up.
• Order and administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and other care team members.
• Coordinate with PCP for patient education and follow-up.
