Matrix Medical Network seeks a Nurse Practitioner to perform Comprehensive Health Assessments at patients' homes or facilities. You’ll review health history, meds, and risk factors, and coordinate with physicians and care teams to identify and close care gaps.

The role emphasizes patient education, timely documentation, and collaboration with home-based teams. A valid NP license in the state of practice is required, with eligibility for prescriptive authority and board certification in a

Competenze

• Must hold NP licensure in practice state.
• Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care.
• Master’s Degree or commensurate experience for NP practice.
• BLS/ACLS or CPR certification required.

Mansioni

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and family members to coordinate care.
• Coordinate with PCP on patient education and follow up.
