Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform in-home Comprehensive Health Assessments, reviewing health history, medications and social factors to close care gaps. Visits occur at members' homes or in Skilled Nursing Facilities, with a focus on improving outcomes and care coordination.

The role supports flexible scheduling with weekend or evening shifts and potential full-time transition. Board-certified NP with state licensure and strong communication is required; drivers

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice to include prescription authority or ability to obtain prescriptive authority.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience preferred, new grads encouraged to apply.
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage requirements for the state of residence.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers as appropriate.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facilities).
