Matrix Medical Network is seeking a Nurse Practitioner to perform in-home comprehensive health assessments, reviewing medical history, medications, and social factors. You will work with a team to ensure high-quality care during visits at members' homes or Skilled Nursing Facilities.

Applicants should hold an NP license, be board-certified, and have prescriptive authority. Flexible scheduling and full-time opportunities with a sign-on bonus are available.

Qualifikationen

• Must have NP licensure and state practice authorization.
• 1 year NP experience preferred; new grads welcome.
• Current BLS, ACLS or CPR certification.
• Valid driver’s license and auto insurance.
• Willingness to travel to member residences.

Aufgaben

• Conduct adult/geriatric assessments including medical history and follow-ups.
• Order/perform point-of-care and lab tests as needed.
• Collaborate with PCPs, case managers, social workers, and family.
• Provide services at members' homes or SNFs.
• Document findings per Matrix guidelines.
