Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments across the United States. You will review health history, meds, and social factors, and document findings per Matrix protocols.

The role emphasizes collaboration with PCPs, case managers, and caregivers, with responsibilities at patients’ homes or SNFs. A current NP license and state prescriptive authority are required.

Qualifications

• Master’s degree or equivalent experience in nursing practice.
• Current NP licensure in state of practice with prescriptive authority if required.
• Board certification in appropriate NP specialty.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license.
• Auto insurance coverage for driving to member homes.

Responsibilities

• Conduct geriatric assessments including medical history and follow-up.
• Administer and order point-of-care and lab testing.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Provide services at members' residences (homes or SNFs).
• Coordinate with PCP on education and follow up.
