Matrix Medical Network is hiring Nurse Practitioners to perform Comprehensive Health Assessments in members' homes or Skilled Nursing Facilities. You’ll review health history, medications, and risk factors, and collaborate with PCPs and care teams to close care gaps.

We offer flexible scheduling, travel reimbursement, and FT opportunities with a competitive pay model. Must hold NP licensure in your state and current BLS/ACLS certification, with 1 year of NP experience preferred.

Qualificações

• Master’s degree or commensurate NP experience and successful NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, AGPCNP, AGACNP, Family, Emergency or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license for the state of residence and automobile insurance coverage.
• Strong computer skills and familiarity with health record software.
• Excellent verbal and written communication skills.

Responsabilidades

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and documentation according to Matrix guidelines.
• Administer and order point of care and lab-based testing as needed.
• Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary.
• Provide services at the member’s residence which can be in their homes or Skilled Nursing Facilities.
