Job summary

Matrix Medical Network is seeking licensed nurse practitioners to perform in-home comprehensive health assessments. Visits can last up to one hour and may occur at homes or skilled nursing facilities, with documentation and follow-up care coordinated with a care team.

Responsibilities include medical history review, testing, patient education, and collaboration with PCPs to close care gaps. Flexible scheduling, competitive pay, and ongoing education support are provided.

Qualifications

• Must hold NP licensure in state of practice.
• Current licensure to prescribe medications if required.
• Board certification in a primary care or geriatric specialty.
• BLS/ACLS/CPR certification current.
• Valid state driver's license and auto insurance coverage.

Responsibilities

• Conduct adult/geriatric assessments including medical history and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and family members.
• Provide services at the member’s residence, including homes or SNFs.
