Job summary

Matrix Medical Network is seeking a licensed NP to perform Comprehensive Health Assessments at members’ homes or SNFs, reviewing health history, medications, and risks. The role provides meaningful impact on care gaps and outcomes with a schedule that fits your availability.

Responsibilities include conducting geriatric assessments, guiding referrals, and collaborating with PCPs and care teams. On-site visits offer flexibility with potential FT transitions and ongoing support.

Qualifications

• Master’s degree or equivalent in nursing and NP licensure completed.
• Current NP state licensure with prescribing authority if needed.
• Board certification in Adult, Gerontology, Family or Emergency NP specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and follow-up.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other staff.
• Coordinate education and follow up with the PCP as necessary.
• Provide services at members' homes or Skilled Nursing Facilities.
