Job summary

Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes, including review of health history, medications, and risks. Assessments may occur in homes or Skilled Nursing Facilities, lasting up to one hour.

Providers will order tests as needed, work with a multidisciplinary team, and coordinate with PCPs for education and follow-up, with flexible scheduling and potential FT opportunities.

Qualifications

• Master’s degree in nursing or commensurate experience.
• Current NP licensure in state of practice.
• AANP/AACN/ANCC board certification in NP specialty.
• BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and automobile insurance.
• Strong computer skills and familiarity with medical records software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatment documentation.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, and other care team members.
• Coordinate with PCP for patient education and follow-up.
