Job summary

Matrix Medical Network seeks clinician providers to perform Comprehensive Health Assessments in members' homes or SNFs. You will review medical histories, medications, and risk factors, and coordinate with PCPs and care teams to close care gaps.

A master’s NP degree with current state licensure and board certification is required. Positions offer flexible scheduling, including weekends, and potential for licensure support and ongoing education.

Qualifications

• Master’s degree or commensurate experience with NP licensure.
• Current NP licensure in state with prescriptive authority or ability to obtain.
• Board certification in Adult, Gerontology, Family, or related specialties.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license; auto insurance coverage as required.
• Strong computer skills and experience with health records software.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnoses.
• Order and administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Educate patients and provide follow-up in coordination with PCP.
• Provide in-home or SNF-based health assessments and documentation.
