Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in‑home health assessments. You will review medical history, medications, and social factors to identify care gaps and coordinate with PCPs for follow-up.

Assessments occur at members' homes or SNFs, with flexible scheduling and support for licensure advancement. Qualified candidates hold NP licensure, board certification, and current BLS/ACLS, with 1 year NP experience preferred.

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult, AG-PCC, AG-AC, Family, Emergency, or Acute Care.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and families.
• Coordinate education and follow-up with primary care physicians.
• Provide services at the member’s residence (home or skilled facility).
