Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform in-home comprehensive health assessments. You will review health history, medications, and risks, and coordinate with physicians and care teams. Assessments take place in members’ homes or SNFs, with documentation guided by Matrix protocols.

The role emphasizes travel across states, with flexible scheduling and potential transition to full-time roles, plus ongoing education and licensure support.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• Willingness to travel up to 80–90% of the time, including overnight travel.
• Valid state driver’s license and auto insurance in residence state.
• Strong computer skills and familiarity with health/medical record software.

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatments.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCP, case managers, social workers and family members.
• Provide services at members’ residences (homes or SNFs).
• Follow Matrix guidelines and ensure thorough documentation.
