Job summary

Matrix Medical Network seeks licensed Nurse Practitioners to perform comprehensive in-home health assessments, review medical histories, and coordinate care with physicians and case managers. Visits are conducted at the member’s home or in Skilled Nursing Facilities, typically lasting up to one hour, with flexible scheduling to match availability.

The role requires NP licensure, board certification, and ability to travel up to 80-90% across states.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• AANP/AACN/ANCC board certification in an eligible NP specialty.
• Current BLS/ACLS or CPR certification.

Responsibilities

• Perform adult/geriatric assessments including medical history and diagnosis.
• Provide patient education, referrals, and follow-up with PCPs.
• Order and administer point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, and care teams to coordinate care.
• Deliver services at patients' homes or in Skilled Nursing Facilities.
