Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, evaluating medical history, medications and risk factors. You will order tests, coordinate with PCPs, and provide education during visits at members' homes or SNFs.

The role offers flexible scheduling with numerous home visits across states, opportunities for licensure support, and a path toward full-time positions with competitive compensation.

Qualifications

• Master's Degree in nursing or equivalent and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult, Adult Gerontology or Family NP specialties.
• Current BLS, ACLS or CPR certification.
• Travel up to 80–90% of time, including overnight.
• Valid state driver''s license and auto insurance coverage.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnosis.
• Administer and order point of care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate patient education and follow-up with PCP.
• Provide services at member residences (homes or SNFs).
