Job summary
Matrix Medical Network seeks a highly qualified Nurse Practitioner to perform Comprehensive Health Assessments in patients' homes or Skilled Nursing Facilities. You will review medical history, treatment plans, and educate patients, following Matrix guidelines and protocols.
The role requires NP licensure in the practice state, board certification in a relevant specialty, and willingness to travel extensively. Flexible scheduling and ongoing education support are provided.
Qualifications
- Master’s Degree and NP licensure in state of practice.
- Board certification in Adult, Gerontology, Family, Emergency or Acute Care.
- BLS/ACLS or CPR certification
Responsibilities
- Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation.
- Administer and order point of care and lab-based testing as needed.
- Collaborate with Primary Care Physician (PCP), physicians, case managers, social workers, family members and other personnel as appropriate.
- Provide services at the member’s home or Skilled Nursing Facilities as needed
