Job summary
Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive health assessments in patients’ homes and SNFs. You will evaluate medical history, medications, and risk factors, and collaborate with PCPs to close care gaps.
We offer flexible scheduling, opportunities for growth, and competitive compensation with field-based travel. Requires NP licensure and prescriptive authority; BLS/ACLS preferred.
Qualifications
- Must be a licensed NP with current state licensure.
- Prescriptive authority or ability to obtain as needed.
- Board certification in Adult, Adult Gerontology, Family or Acute care.
- Current BLS/ACLS certification.
- 1 year NP experience preferred; new grads welcome.
- Valid state driver’s license and auto insurance.
- Strong computer skills and familiarity with EMR systems.
- Excellent verbal and written communication.
Responsibilities
- Perform adult/geriatric health assessments and document findings.
- Review medical history, diagnoses, medications, and risk factors.
- Order and administer point-of-care and lab tests when indicated.
- Collaborate with PCPs, case managers and other care team members.
- Provide services at members’ residences (homes or SNFs).
