Job summary

Matrix Medical Network hires NPs to perform Comprehensive Health Assessments in members' homes or Skilled Nursing Facilities, reviewing health history, medications, and risks during visits that last up to an hour.

You’ll collaborate with physicians and care teams, receive flexible scheduling, travel reimbursements, licensure support, and career advancement opportunities. Full-time roles offer competitive pay, onboarding, and a preceptor-mentored environment.

Qualifications

• Master’s degree or commensurate experience required.
• Current NP licensure in state of practice with prescriptive authority as needed.
• AANP/AACN/ANCC board certification in adult, gerontology, family, emergency, or acute care.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance as per state requirements.
• Strong computer skills and familiarity with health records software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP on patient education and follow-up.
