Job summary

Matrix Medical Network provides in-home comprehensive health assessments conducted by licensed NP providers. Each visit lasts up to one hour and covers health status, history, medications, and social determinants.

You will order tests as needed and collaborate with physicians and care teams to close care gaps. We offer flexible scheduling, competitive pay based on assessments completed, mileage reimbursement, 401(k), and ongoing education support.

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• AANP/AACN/ANCC board certification in Adult or Family/Gerontology specialties.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance for residency state.
• Proficiency with health-record software and digital tools.

Responsibilities

• Conduct adult/geriatric in-home health assessments up to one hour.
• Review medical history, medications, and social factors; document per Matrix guidelines.
• Order/perform point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, and care team for follow-up and education.
