Matrix Medical Network is hiring nurse practitioners to perform comprehensive in-home health assessments, reviewing medical history, medications and risk factors. Assessments occur in members’ homes or SNFs with a strong emphasis on close care coordination and documentation.

Providers work a flexible schedule with weekend and evening options, earning pay based on the number of assessments completed. Training, licensure support and continuing education are offered, with full-time opportunities

Formación

• NP licensure and board certification (AANP/AACN/ANCC)
• Current BLS/ACLS or CPR certification
• 1 year NP experience preferred; new grads welcome
• Valid state driver’s license and auto insurance coverage

Responsabilidades

• Conduct adult/geriatric assessments including medical history and treatments
• Order and administer point-of-care and lab tests as needed
• Collaborate with physicians, case managers and other care team members
• Coordinate with PCPs on patient education and follow-up
• Provide services at the member’s residence (home or SNF)
