Job summary

Matrix Medical Network, a national in-home health assessment provider, seeks nurse practitioners for in-home comprehensive health assessments. You’ll evaluate health history, medications, and risk factors, then coordinate with physicians and care teams to close gaps in care.

Perform assessments in members' homes or SNFs, with flexible scheduling and potential for full-time opportunities. A strong clinical background, licensure, and communication skills are essential.

Qualifications

• Master’s degree and NP licensure in state of practice with prescriptive authority or eligibility.
• Current board certification in Adult/Family/Gerontology acute care or related field.
• Active BLS/ACLS or CPR certification and clean driving record.

Responsibilities

• Perform comprehensive in-home health assessments (CHA) for adults and geriatric members.
• Review medical history, diagnoses, medications, and adherence; document per Matrix guidelines.
• Coordinate with PCPs, case managers, and family/caregivers for follow-up plans.
• Order/interpret point-of-care and lab tests as needed; provide health education.
• Travel to members' homes or SNFs; ensure documentation and care gaps closure.
