Job summary

Matrix Medical Network seeks licensed NP providers to perform Comprehensive Health Assessments in members’ homes or Skilled Nursing Facilities. You will review medical history, medications, social factors, and document findings per Matrix guidelines to close care gaps.

Responsibilities include administering tests, collaborating with PCPs and care teams, and delivering education with follow-up. Flexible scheduling, mileage reimbursement, and full-time opportunities with benefits are provided.

Qualifications

• Master’s Degree or commensurate experience and satisfactory NP licensure.
• Current NP licensure with prescriptive authority as needed.
• Board certification in relevant adult-gerontology/primary care specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads may apply.
• Valid driver’s license and auto insurance for residence.

Responsibilities

• Perform adult/geriatric assessments including medical history, diagnosis and treatment, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Educate patients and follow up with PCPs.
• Provide services at the member’s home or Skilled Nursing Facilities.
