Matrix Medical Network is hiring nurse practitioners to perform comprehensive in-home health assessments. You will review health history, medications, and social factors to identify care gaps and improve outcomes.

Responsibilities include performing assessments at members’ homes or SNFs, coordinating with primary care providers and care teams, and documenting clearly per Matrix guidelines. Flexible scheduling and opportunities for full-time transition are offered.

Formación

• Must hold NP licensure and prescriptive authority where required.
• Board certification in Adult, Gerontology, Family or Emergency NP specialties.
• Current BLS/ACLS or CPR certification required.
• 1 year NP experience preferred; new grads welcome to apply.

Responsabilidades

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment planning.
• Order and administer point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers and caregivers for coordinated care.
• Provide in-home assessments at members' residences or SNFs.
• Document findings clearly and in accordance with Matrix guidelines.
