Job summary

Matrix Medical Network seeks Nurse Practitioners to perform in-home health assessments. Each visit lasts about an hour and covers medical history, medications, social context, and potential gaps in care to tailor treatment plans.

You’ll collaborate with PCPs, case managers, and family members to improve health outcomes. We offer flexible scheduling, competitive pay per assessment, mileage reimbursement, and licensure support.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure with prescriptive authority.
• Board certification in Adult/Family/ Gerontology specialties.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric assessments including medical history and documentation per Matrix guidelines.
• Order and administer point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, and family members.
• Coordinate with PCPs on patient education and follow-up.
• Provide services at the member’s residence (home or SNF).
