Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments across our national network. You will review health history, medications, and risks to provide actionable insights that close care gaps and improve outcomes.

Responsibilities include conducting geriatric assessments, ordering tests, collaborating with PCPs, and delivering care at home or in Skilled Nursing Facilities. A valid NP license and prescriptive authority are required.

Formación

• Must hold NP license in state of practice with prescriptive authority
• Board certification in Adult, Adult Gerontology Primary Care, Family, or Emergency nursing
• Current BLS, ACLS or CPR certification
• 1 year of NP experience preferred, new grads encouraged to apply
• Valid state driver’s license and auto insurance coverage
• Proficient with electronic health records and health IT systems
• Excellent verbal and written communication skills

Responsabilidades

• Conduct adult/geriatric health assessments including medical history and diagnoses
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers and caregivers
• Coordinate with PCP for patient education and follow-up
• Provide in-home health assessments at members’ residences including SNFs
