Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform comprehensive in-home health assessments for members in their residences, including homes and Skilled Nursing Facilities.

Each visit lasts up to an hour and includes medical history review, medication adherence, and risk assessment, with coordination with PCPs and care teams.

We offer flexible scheduling, substantial earning potential based on completed assessments, and support for licensure and ongoing education.

Qualifications

• Master’s Degree or commensurate experience and satisfactory NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult/Nurse Practitioner specialties.
• BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, and follow-up.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers, and family caregivers as appropriate.
• Provide services at members' residences, including homes and SNFs.
• Document clearly according to Matrix guidelines and protocols.
