Job summary

Matrix Medical Network is seeking experienced Nurse Practitioners to perform comprehensive in-home health assessments. You will review medical history, medications, and social factors to identify care gaps and coordinate follow-up with primary care physicians.

Assessments occur at members' homes or SNFs, offering flexible scheduling and competitive pay. Position requires NP licensure in practice state, prescriptive authority, board certification, and current BLS/ACLS.

Qualifications

• Master’s NP required and active state license with prescriptive authority when needed.
• Current NP licensure in state of practice with prescriptive authority.
• AANP/AACN/ANCC board certification in relevant NP specialties.
• Current BLS/ACLS certification.
• 1 year NP experience preferred; new grads welcome to apply.
• Valid state driver’s license and auto insurance coverage in home state.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnoses, treatments, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, family members, and care teams.
• Educate patients and coordinate follow-up with PCPs.
• Provide services at members’ residences, which may be in homes or Skilled Nursing Facilities.
