Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform in-home comprehensive health assessments. During visits, you review health history, medications, and social factors to identify care gaps and improve overall well-being.

Qualifications

• Master's degree in nursing or commensurate NP experience.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care nursing.

Responsibilities

• Conduct adult/geriatric assessments with medical history and diagnoses.
• Administer and order point-of-care and lab-based tests as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate care with PCP for patient education and follow-up.
