Job summary

Matrix Medical Network seeks licensed Nurse Practitioners to perform in-home comprehensive health assessments. Each visit can last up to one hour and includes review of medical history, medications, and social factors to identify health gaps.

You’ll work with physicians, case managers, and family members to coordinate care and deliver education at the member’s residence or SNF. Offers flexible scheduling with a nationwide service model, competitive pay based on assessments completed, and

Qualifications

• Master’s Degree in Nursing and NP licensure.
• Current NP licensure with prescription authority.
• Board certification in NP specialties (Adult, Gerontology, Family, Emergency, etc).
• Current BLS/ACLS or CPR certification.
• At least 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Perform adult/geriatric health assessments during home visits.
• Review health history, medications, and social determinants; document per Matrix guidelines.
• Coordinate with PCP, case managers, social workers, and family members.
• Provide services at the member’s home or Skilled Nursing Facility as needed.
• Order point-of-care and lab-based testing when appropriate.
